I've never seen it spawn outside the bottom half of the map, but I've only seen it happen twice since I tend to get lucky with dungeon generation.

I'm pretty sure those openings are always on the bottom of the dungeon map, but I would have to collect more examples to be absolutely certain. I've seen it happen a number of times, particularly running one dungeon and having multiple floors have the hole in them, but it's difficult to say I've never seen at least one case where the hole was not the south-most thing in the dungeon.
