Back-attacks on mobs seem to proc on their sides, and their back.
I haven't tested it yet, but, I imagine the same goes for players.
EDIT: I have tested it. Yes, it applies to players. Oh the pain.
UPDATE: It seems getting bumped from your back diagonals causes the player to get the back attack instead.

This has been corrected. Not sure I understand the last bit though.

To clarify, a monster moving into -you- can give you the back attack (as in you get the free round, not them). I can't seem to reproduce it beyond having one person in my party to stand behind me, and waiting for the monster to approach me while I face north.
I've had it happen while facing other directions, so, I'm not entirely sure which parts of the monster/player are causing that to happen.
I'd imagine this fix should stop that from happening, though.
