I came across this oddity while going dungoning. It was the very first floor of an level 31 waterpool but it generated with no way/doors at all.

These happen a fair bit. The most common version is bandit dens with the path through them being blocked by a jail cell or something else impassible. (Well, most common to my knowledge anyway)
