Those two squared rows are dense.
The 'Ghetto Healer NPC' part is just a small gimmick idea. Can you add a healer there that have a 50% chance to fuck up their heals and injure you with a random physical ailment? (But still recover HP/FP as per usual) And for example on wounds, they could range from small bruises to severe cuts and such.
The reason for this is simple. This spot looks like a great place for some underground dog fights! Or just one hell of an uncommon hangout. And slum-like areas should get more love. Maybe some conflict can arise too, to make the world a little less stale?

You are not supposed to go down the ladder. It is just a dead end down there. The sewers was going to be larger, but I cut most of it just to get Cellsvich out.
The dense tiles on the left side was for people to not walk through the fence and get on the bottom floor under the ladder, cause as stated, it's a dead end.
