I noticed the same problem with crafting tomes and the Enchanting
legend extension, just today with 1.04 already live. My total level in
Enchanting was 7, and crafting a whole bunch of difficulty 3 tomes
gave me some with 1 quality and some with 3, but never any 4's or
2's.
