That's similar and direct opposite of an issue I had a while time ago of being able to take these quests twice, I suggest you ask Dev to rewind your save.
For instance, this is what I'm referring to:

Did anything odd happen when you relog? (Like having 1/(MaxHP) and / FP, which was my case in particular) Did you relog close to reboot/at reboot? Disconnect or crash? Closed the game by the X?
