To explain the joke simply. In most joking cases 'Thick' can be used to describe body parts of a sexual nature, albeit the Buttocks, the males lower spear, thighs or what have you. The older definition of Thick means thick headed, dense, or slow. In this case the older term was used but then in the context Raigen just kind of went in a completely different direction with his wording as 'Extra Thick' has become a meme now on the internet, though icly he didn't mean it like a meme. So it's funny outside of the game. Or inside if you think of 'thick' sexually.
