Whenever using cutthroat, and I assume a few other skills whilst dual wielding, the offhand weapon seems to love taking precedence over the main hand. Which can cause issues with cutthroat and a shuriken whenever you really meant to actually cutthroat and not proc a spirit mirror.

Why not switch the order, then?

Because some skills, like Repel, seem to take main-hand precedence over off-hand precedence due to the 'any weapon type' which causes really weird issues.

Didn't Tamaki talk about Setsuna doing this every time he used Sidecut?

Try unequipping and re-equipping the sub weapon.

No matter what weapon you have in main-hand or what weapon you equip first, you appear to .always. use the weapon that has the most power for weapon-specific skills.
Sidecut was using my off-hand Katana at 18 power instead of my mainhand narcus at 13 power.
Whenever I swapped hands for them, it still used the Katana over the Narcus. This appears to be the case with the initial report too.
