Sneaking makes monsters unaware of you if they are melee based and you're not standing next to them, but if they're a mage like Shamans, or have ranged attacks they still attack you even when you're a few squares away. Even Gorgon's Run Down can still get you if you're on line with them.

So basically, you're not 100% undetected.

The description literally says "if you're not in attack range."

Ranged attacks count as attack range. This seems to be working as intended?
