When you have a secondary weapon with an Item Regalia applied (in this case I used two shurikens with the same custom icon), your sub-attack and the secondary twin dance attack don't display the custom icon. The default icon is shown in both the attack and icon next to the weapon's name in the chatbox. This also applies even if both of your weapons have an Item Regalia applied.

The only part of this that was actually a bug was the icon part; it was being caused by attacking with a weapon in the chat log and then changing its appearance. The old one was being cached by the browser and reused. I have made an adjustment to fix that.

As for the attack animation, that wasn't a bug, but I have changed the behavior for the shurikens to use the weapon icon if it's customized.
