This bug can only occur for us when we are venturing into lairs, and dungeons that is spawned throughout the region.
But if the dungeon, or lair is not completed in specific time. It'll start to suffer a despawn period, in which the dungeon promptly disappear. In this case, the dungeon despawned with us inside when we went down a floor.

Yes. This is exactly what happens. When the dungeon despawned, we were shown a pitch black. No escape, not even Alga Plume can be used. So the moderator told us to go, and see if we can relog.



We appeared back in the Professor Pink's office after our relogging.





So I would assume this bug have relations with the dungeons expiring with the players inside, and then entering a pitch black area where anything cannot be used to escape. The solution would be to re-log, and appear in Professor's office where red symbol in bottom of the room is.

I dare say it's as random as the basic-hit-on-hotbar-dun-work bug. But in this case, people can just lol warp off on the pentagram teleporter.
